if you're referring to the coach who took you out of an important game and then forgot to put you back in... i probably would 'forget' to talk to him at all. "Oh, i'm sorry, i didn't know you were here...".

But if yer aksin'me, I'd call him coach. I was never pulled from a game though...
The proper term would be "Mr/Ms Coach" This person tried to teach you not only the fundamentals of the sport but also about life. Don't cheat or cut corners; Don't bust up someone's face because you lost. Yes, my son, you must go out and play against the 12 year old 7th grader who has hair under his armpits. Stop whining. Unfortunately, Mr/Ms coach was not always successful with you lame brains. Trust me on this, I have experienced it!!
I call my old coach Dorno the Porno.
